The problem

PPE procurement is where spec drift happens silently.

Safety procurement looks simple. It is not. A Cat 2 arc-flash jacket and a Cat 3 arc-flash jacket look identical to a purchasing system but represent the difference between meeting and missing your incident-energy classification. Most procurement workflows do not catch this until incident review.

Cut-level confusion costs hands.

"Cut-resistant gloves, blue, size L" is what shows up on most reorder lists. Then the cut level gets substituted from A3 to A2 because the distributor was out of stock. The plant safety officer finds out at the next incident.

Hi-vis class shifts with the substitution.

Class 2 hi-vis is required for highway and road work. Class 3 is required for night / low-light. Class 1 is parking lot only. Vests look similar; specs are different. Substituting on the line item is common and wrong.

FR fabric weight matters.

NFPA 2112 coveralls in 4 oz fabric vs. 9 oz fabric are both compliant but have very different arc ratings. Specifying just "NFPA 2112 FR coveralls" leaves room for substitution that downgrades real protection.

Inspection records die in the field.

Fall-protection harnesses are required to have inspection records. In practice, the paper trail breaks at the third or fourth user. Most operators rebuy harnesses on a 3-year cycle just to have known-history equipment.
